ISMED-CLIM is a Horizon Europe Innovation Action that aims to strengthen climate resilience and protect public health across Mediterranean countries by developing and validating innovative solutions that mitigate climate change-related health risks. The project integrates climate science, environmental monitoring, epidemiology, artificial intelligence, early warning systems, citizen engagement, and healthcare innovation to improve preparedness against heat stress, air pollution, vector-borne diseases, and other climate-related hazards. Through Living Labs and demonstration activities implemented across eight pilot sites in five Mediterranean countries, ISMED-CLIM develops and validates climate-health adaptation solutions, including personalised heat-health alert systems, climate services, digital decision-support tools, and regional early warning systems. The project combines environmental observations, climate forecasts, exposure assessment, health data, and socio-economic analyses into interoperable digital platforms supporting healthcare professionals, policy makers, local authorities, and citizens. Ultimately, ISMED-CLIM contributes to the implementation of the EU Mission on Climate Adaptation by enhancing health system resilience and supporting evidence-based climate adaptation policies across the Mediterranean region.
